Graduate Appointment App

Hi all!
As I have been working on the application for Univ. Illinois Chicago, there is a section labeled "Graduate Appointment App.", when I click the link, it leads me to a document upload page without any instructions. Anyone have any idea what this section is requesting? Most grateful!
