I'm a senior undergrad at a fairly unknown state school. My GPA is only 3.2 maybe 3.3 after this Fall. I did an REU in quiver path algebras which was an okay paper but nothing crazy. I'll get three rec letters from professors, one from MIT and one I did the REU with. My other letter writer I'm doing a reading tyoe thing with on Schubert calculus.
My GRE scores are Q: 163, V: 147, W: 4.5 and I've yet to take the subject. I have a feeling I won't make it to 700 score because of the time I have left. I've been a tutor for a company and privately for almost 3 years, and I'm a grader for calc II and diff eq. I'm also in a graduate algebra course which I should make an A in.
My question is: given the above applicant profile, is it realistic to get accepted to, say, a top 50 or 60 program? Thanks for your replies.

Sure why not. Don't sell yourself short. I say apply to several top 50/60 programs. You will never know if you don't try.
